If you want to use MyRKZ for someone else, or if your caregiver needs access to your MyRKZ, you can request authorization.
Below you will find how to apply for authorization per age category.
A parent or guardian can request authorization by completing the (paper) authorization form.
Children aged 0 to 12 do not have to give their own permission for this.
After assessment and permission from the treating physician, File Management ensures that the child's file becomes visible in the care portal of the parent/guardian.
No DigiD authorization needs to be requested. The parent/guardian logs in with their own DigiD. The second option on the login screen.
After that, the child's medical file can be opened.
The authorization form can be found here:

Once a child turns 12, he or she may participate in decision-making and the child must co-sign the requested (paper) authorization.

From the age of 16 it is no longer possible to apply for a new authorisation.
The child can then apply for a DigiD to gain access to his or her medical data.
If the parents want to inspect the file, the child can arrange this themselves by giving authorization.
This is done via the DigiD environment.

DigiD: DigiD – Request Authorization.
You can log in via option three: Authorize DigiD.
