Online PROMS is a system that allows healthcare organizations to send questionnaires to you. Your answers give your healthcare provider more insight into your health, treatment, and experiences with care. This allows the care to better align with your situation.

You will receive the questionnaires via e-mail or via the Patient Journey App. You open the questionnaire via the button in the message, answer the questions, and click on Complete to send your answers.

Sometimes you must first a code complete this before you can open the questionnaire. You will receive this code via email. Can't find the code? Then also check your folder with unwanted email.
