How do I fill out a questionnaire via My Radiotherapy Group?

  • click on 'Questionnaires'
  • click on 'To fill in' to fill out the questionnaire

Log in to your My Radiotherapy Group.

2. Open questionnaires overview

After logging in, you will arrive at the homepage.
Click at the top of the page on 'Questionnaires'.

3. Open or view questionnaire

You are now on the page where all the questionnaires that have been prepared for you are located.

Here you see two headings:‘Questionnaires’ and‘Completed questionnaires’.

If you want to view a questionnaire you previously completed, click on ‘'To look at'’.

If you still need to complete a questionnaire, click on ‘'To fill in‘'‘ next to the questionnaire.

Should you stop filling in the form midway and leave My Radiotherapy Group, you will lose the entered answers.
